This “Gifts of Health” program is designed to reward you as a referrer with credit you may use toward ongoing or future medical care.
Our Gifts of Health and Bonus Dollars Rewards Program is a great way for you to show others the benefits you have gained through our services, and also make your treatment more affordable in the future.

Getting Started is Easy
Simply complete the Referral Rewards Program information and the person you have thoughtfully referred. Our staff will enter this information and keep track of your referrals and “Bonus Bucks”.
Not only do you get Bonus Dollars from the monies collected from your initial referral, should your referred patient refer patients as well, you also are rewarded. You earn Bonus Dollars for everyone that has paid for treatment do to your initial referral.
Rewards Accumulate Quickly
The maximum Bonus Dollars Referral Rewards percentage is 15 percent. For each initial patient refer, you will receive 7.5% of all monies collected. Your secret code assures that any additional referrals will be accurately traced and added to your Bonus Dollars account. Once a patient that you referred refers another patient, you then receive 3.75%.
Here’s an example of how this works:
John refers Mary. Mary’s treatment costs $500. John receives 7.5% of that amount, which is $37.50.
Mary then refers Mark. Mark signs up for our yearly wellness program ($1595). Mary receives 7.5% of $1595, or $119.63, and John, the initial referrer, receives 3.75% of $1595, which is $59.81.
Mark then refers Dr. Smith, and Dr. Smith receives $650 of care. Mark gets 7.5% or $48.75: Mary receives 3.75% or $24.38: and John receives 1.9% or $12.19
